The Role The Senior Clinical Nurse (SCN) aims to improve health outcomes and assist clients to remain living at home, by providing safe, responsive and cost efficient nursing care.
The SCN assists the care team by authorising delegation of care and managing RN’s, EN’s, Support Workers & Administration Staff.
The Senior Clinical Nurse will be responsible for the initial establishment and/or review of the community nursing or home care plan following a comprehensive standardised assessment.
The role also includes provision of direct client care, highlighting the need for referral to multidisciplinary or specialty services, and providing assistance with training to RN’s, Enrolled Nurses, support workers, Clients and their Carers.
Skills and experience Minimum 3 years experience as a Registered Nurse A current Registered Nursing qualification and sound clinical skills appropriate for complex care needs.
Proven experience in a similar role within the community, aged or disability care environment High level of communication skills that display a commitment to care and empathy for clients and staff Strong organisational, problem solving, conflict resolution and time management skills Outstanding attention to detail Ability to work in a high pressure team environment to meet deadlines Strong background in client relationship management Use of Microsoft Office and an ability to learn new systems related to rostering and client management Understanding of the direct care market Background in business development and/or sales Day to Day Duties Provide responsive, individualised services and support for each person and family Utillising knowledge of clinical outcomes to initiate operational and clinical changes Initiating and coordinating discharge and transfer to other services or programs both internal and external to Programmed Care Network and liaise with clients, organisations and the community Ensure Programmed Care meets all clinical and healthcare regulatory standards and requirements Identify and capitalize on new growth opportunities Supporting our customers to achieve their goals Accurate assessment and review of client care including; clinical suitability for staff delegation, managing health & issues and liaison with hospital discharge planners.
