Key Responsibilities
Daily Living Support
• Assist with personal care, promoting independence, privacy, and choice.
• Support residents during mealtimes according to individual needs.
• Make beds and ensure fresh linen is always available.
• Return and neatly store laundered clothing in residents' rooms.
Health Monitoring & Documentation
• Assist with nursing care following individual care plans.
• Observe and record changes in residents' wellbeing; report concerns promptly.
• Maintain accurate daily notes, care assessments, and care plans.
Communication & Advocacy
• Build trusting relationships with residents and families; offer companionship.
• Liaise with GPs, allied health, and colleagues to ensure coordinated care.
• Uphold residents' rights to privacy, confidentiality, and self-determination.
Teamwork & Culture
• Contribute to a positive team culture and support colleagues.
• Participate in meetings and continuous improvement initiatives.
• Promote inclusion, respect, and a harassment-free workplace.
Safety & Compliance
• Follow WHS policies; identify and mitigate risks.
• Comply with Aged Care Standards and legislation.
• Report hazards, incidents, and complaints promptly.
• Adapt positively to changing service needs.
Learning & Development
• Engage in ongoing training and professional development.
• Apply best practice and policy updates to enhance care.
Selection Criteria
Essential
• Minimum 1-year full-time experience in care or support.
• Diploma or qualification in Aged Care, Individual Support, Nursing, or related field.
• Current National Police Check (or willing to obtain).
• Valid driver's licence and reliable transport.
Compliance
• Up-to-date flu and COVID-19 vaccinations.
• Signed declaration regarding overseas criminal history (if applicable).
• NDIS Worker Screening Clearance (within 5 years) – desirable.
