Qualified Social Worker, Knowsley Metropolitan Borough Council, Adults Hospital Team - Must be a Qualified Social Worker with a minimum of 2 years+ UK Social Worker experience within an Adult Social Work team.

The team works extremely well together to ensure the safety of vulnerable adults under the close supervision of a supportive Team Manager.

The responsibilities will include:

1. Support the discharges from the bed-based intermediate care unit working alongside the Reablement/Rapid Response team
2. Hospital experience would be useful
3. Care Management
4. Review Care Packages
5. Adult Placement
6. Report writing
7. Home Visits
8. Liaise with other professionals
9. 36 hours per week (flexible by negotiation)
10. 2 Month Temporary Contract
11. Start date subject to DBS and reference clearance

The successful candidate will need to hold a Dip SW or equivalent, must be registered with the HCPC, have a current DBS, and be a car driver.
