Non-Medical Case Manager and Benefits Counselor Non-Medical Case Manager and Benefits Counselor Non-Medical Case Manager and Benefits Counselor Position: Non-Medical Case Manager and Benefits Counselor Department: HIV Prevention and Care Location: Minneapolis, Minnesota Salary: $35,000 to $45,000, annually Duration: Permanent, Full-time Hours: Monday – Friday, 8am to 4pm (occasional evenings and weekends) Reports to: HIV Program Manager For 34 years, the Indigenous Peoples Task Force has been at the forefront of closing the health equity gap for Native American and other marginalized communities impacted by HIV/AIDS across the Twin Cities and greater Minnesota. Based on Indigenous values and ways of knowing, our programs have grown to include services that promote integral role of and return to the centrality of first medicines, the use of cultural arts to support youth development, and programs that foster food security in ways that reconnect Indigenous and other peoples to the land and waters. Job Summary Working independently the non-medical case manager and benefits counselor collaborates with clients to provide short to medium term services that address negative impact of social determinants of health for people living with HIV: including but not limited to housing, employment, and financial assistance and benefits program. Using evidence-based interventions, psychosocial techniques, and an integrated approach to HIV care and treatment, the non-medical case manager and benefits counselor enhances each clients’ ability to sustain HIV medical care and treatment. Builds and actively maintains a caseload of 30 clients. Minimum Qualifications Bachelor degree with a working knowledge of HIV and STI modes of transmission, lifecycles, risk factors, harm reduction strategies and best practices. Two to five years of proven experience managing a dynamic caseload through application of case management and other social service best practices. knowledgeable of and experience with Indigenous ways of knowing and culture. And experience collaborating and communicating with diverse stakeholders. Preferred Qualifications Bachelor’s degree in public health, health education, sociology, psychology, social work, or any other closely related field(s). knowledgeable of and experience with Indigenous ways of knowing and culture Knowledge of the National Ending the Epidemic, Sexually Transmitted Infections, and the Ending HIV MN strategies Working knowledge of state and federal benefits systems including but not limited to SSI, SSDI, Medicare, and Medicaid Familiarity with Ryan White Services systems, processes, and standards Working knowledge of and experience with navigating the HIV care continuum Experience providing HIV linkage to care services Experience collaborating with Partners Services to reengage People Living with HIV in care and treatment Certified to provide HIV counseling, testing, and referrals services in Minnesota Experience with group facilitation and dynamics Experience with social media networking and outreach Excellent verbal and written communication skills Proficient with Microsoft Office Suit and database management Must possess and maintain a valid driver’s license Must pass a criminal background check Salary ranges between $35,000 to $45,000, commensurate with experience and qualifications.
