In this role, you will be the bridge between clients and the support they need to thrive. You will assess needs, connect people with resources, and develop meaningful pathways toward independence.
Key Responsibilities
Client Assessment and Support Planning
• Conduct interviews and assessments to understand clients' circumstances, needs, and long-term goals.
• Develop, implement, and review individualised support, educational, and training programs that build skills and independence.
• Monitor client progress, documenting outcomes and adjusting plans collaboratively.
Community Connection and Resource Navigation
• Assess community needs and available resources across health, welfare, housing, employment, and training.
• Refer clients to appropriate external agencies, ensuring seamless coordination of support.
• Prepare funding submissions and detailed reports for government bodies and partner organisations.
Advocacy, Liaison and Community Education
• Build relationships with community groups, welfare agencies, government bodies, and private businesses to address local issues and promote resources.
• Advocate for clients' rights to choice, access, and self-determination.
• Develop community awareness initiatives that reduce barriers and educate the public about available supports.
Specialised Support Across Diverse Settings
• Support families, including education and care for children and individuals with disability in group housing and institutional settings.
• Assist young people to resolve social, emotional, and financial challenges.
• Supervise offenders on probation and parole when required, supporting rehabilitation and community reintegration.
Documentation and Compliance
• Maintain accurate client records, case notes, and progress reports.
• Contribute to service improvement initiatives and team reflective practice.
• Uphold NDIS Practice Standards, legislation, and quality guidelines.
Selection Criteria
Essential Experience and Qualifications
• Minimum one-year relevant experience in community services, social work, or disability support.
• Diploma or tertiary qualification in Community Services, Social Work, Welfare, or related discipline.
• Current National Police Check (or willingness to obtain).
• Valid driver's licence and reliable transport.
Compliance
• Up-to-date flu and COVID-19 vaccinations.
• Signed declaration regarding overseas criminal history (if applicable).
• Current NDIS Worker Screening Clearance (within 5 years) – highly desirable.
